*NEW PLAYERS ONLY*: must supply a birth certificate to be copied and put on file. Care Card is acceptable. .

Registration is first come, first on the list. Returning players only take precedence on the first sign up date, after that time all registrations are time and date stamped to ensure fairness in roster placement if there are wait lists at any level.

All forms must be accompanied by all cheques required.

You will need 3 cheques:

1. reg fee (dated: date of registration)
2. equipment deposit (dated: Nov 15th, 2011 : $350.00)
3. volunteer deposit (dated: Nov 15th, 2011 : $50.00)
